Critical fix: Migration 015 exists on disk but was not registered in db.rs. All beads referencing "migration 015 for mr_file_changes" corrected to migration 016. bd-1oo retitled to reflect dual responsibility (register 015 + create 016). bd-2y79 renumbered from 016 to 017.
